A particularly important type of tumor drug resistance, termed multidrug resistance (MDR), is manifested by cross-resistance to a number of structurally and functionally unrelated lipophilic drugs. This type of resistance has been shown to be the result of decreased drug accumulation in resistant cells due to energy-dependent drug efflux. Acquisition of the MDR phenotype in human cells is the result of expression of the MDR gene, which encodes a 170-kDa membrane glycoprotein, called P-glycoprotein, believed to function as an efflux pump for various hydrophobic compounds. MDR gene expression is frequently observed in different human tumors, both untreated and treated with chemotherapeutic drugs, as well as in some normal tissues. Some tumor types known to be intrinsically unresponsive to chemotherapy, such as renal cell or colon carcinomas, frequently show high levels of MDR1 expression prior to drug treatment.
